The Sharing Health Care Initiative focuses on people over 50 years (or over 35 for those of Aboriginal and/or Torres Strait Islander descent) who have one or more of the following chronic or complex conditions: cardiovascular disease; diabetes; arthritis; osteoporosis; respiratory disorders; or depression, when this occurs with another condition.

The projects undertaken are designed to improve the health-related quality of life and the use of the health care system for people with chronic diseases, particularly those with more than one condition.

Projects are varied and are conducted in urban areas, rural communities and also in Aboriginal and/or Torres Strait Islander communities. The results of the projects are being evaluated and will be communicated to a wide range of health professionals so treatment for these conditions can be improved throughout the community.
