Community Aged Care Packages (CACPs) are designed to be used if your care needs are more complex and you need help with bathing, meals, shopping and getting around. Each package is individually tailored and you can get access to a variety of services which can help you to continue to live in your home.

To access a CACP you first need to be assessed by an Aged Care Assessment Team. Then, if a CACP is appropriate for your needs, you will be referred to a service coordinator.

The Department of Health and Ageing website has up to date information on CACP subsidies and fees.
