If you have a chronic illness and complex care needs, two services aimed at helping to coordinate your care more effectively are available to you.

A care plan is an action plan which sets out the care to be provided to you by your medical practitioner and at least two other health or community care providers.

A case conference is a meeting held by your medical practitioner and at least two other health or community care providers to identify your immediate care needs and the services you require.
